Sensibilidad al gluten

Gluten sensitivity is a systemic autoimmune disease that occurs in genetically susceptible individuals on ingesting gluten. It can appear at any age, then becoming a permanent condition. It is more frequent in women, as happens with other autoimmune diseases. Celiac disease is the intestinal form and the most important manifestation among a set of gluten-induced autoimmune pathologies that affect different systems. Neurological manifestations of gluten sensitivity, with or without enteropathy, are also frequent, their pathogenesis including an immunological attack on the central and peripheral nervous tissue accompanied by neurodegenerative changes. The clinical manifestations are varied, but the most common syndromes are cerebellar ataxia and peripheral neuropathy. Finally, gluten sensitivity is associated to a varying degree, with other complex diseases and could influence their evolution. The early detection of cases of gluten sensitivity with neurological manifestations and subsequent treatment with the gluten-free diet could provide remarkable benefits to the patients.

This research study is just another study of an ever growing list of medical research documenting the existence of gluten sensitivity in the absence of celiac disease. Why is this important? Most doctors don’t know the difference. Most doctors aren’t aware that gluten can contribute to hundreds of symptoms, syndromes, and diseases. Many of these symptoms manifest not in the gut, but in other tissues and organs.

Muscle and joint pain

Sensible al gluten pero no celiaco? Cuidado! La probabilidad de tener cancer es mayor.

Ireland is thought to have one of the highest incidences of celiac disease in the world. Considering this fact, a research team from Belfast conducted a population-based retrospective cohort study to determine the incidence of malignancy and mortality in patients with celiac disease and gluten sensitivity. The researchers reviewed the records of 13,338 people in Northern Ireland whose blood had been tested for increased antibodies to endomysium (highly suggestive of celiac disease) and gliadin (indicative of gluten sensitivity). They then analyzed the medical and death records of the patients who tested positive (490 for endomysium and 1133 for anti-gliadin) and compared the rates of malignancy and mortality against the incidences in the general population.

The team found that patients with celiac disease or gluten sensitivity had higher mortality rates than the general North Irish population.

In addition, they found that the incidence was higher in patients with celiac diease for:

    Breast cancer
    Prostate cancer, and
    Liver cancer

The incidence was higher in patients with gluten sensitivity for:

    All malignancies,
    All malignancies diagnosed 6 months after diagnosis,
    All malignancies diagnosed 12 months after diagnosis,
    Gastrointestinal cancer
    Small intestine cancer
    Lung cancer

This association persists more than one year after diagnosis in patients testing positive for anti-gliadin antibodies”.
According to the authors, the fact that patients with gluten sensitivity had a higher occurrence of malignancy is an interesting finding and necessitates further study to determine its cause. The authors also noted their surprise at the high number of people who tested positive for gliadin antibodies, adding “some of these patients may be gluten sensitive but as yet have no damage to the endomycial muscle. If this is the case then this is the first study to investigate the risk of malignancy and mortality in this group of patients”.

Podria no ser el gluten el causante de la sensibilidad al gluten no celiaca?

They compared immune responses to traditionally-grown wheat with high-output varieties. Their results found a link between gluten sensitivity and the Adenosine Triphosphate Amylase (ATI) protein, which is found in larger quantities in high-output wheat.

ATIs are natural insect repellents and modern farming techniques grow high ATI-containing crops to increase their yield. This could explain why gluten sensitivity has been on the rise.

But study author Detlef Schuppan is cautious about interpreting the results. “The whole thing is very new,”
“We need more data from clinical trials to deduce the consequences for patients.”
